Course Description:

This program will provide an interprofessional (IPP) perspective on understanding the impact of pharmacy and medication management on the rehabilitation of the medically complex patient.  A review of approaches to pharmacology and medication management to reduce the potential for hospital readmission will be conducted.  The role of physical, occupational, and speech therapy practitioners in supporting pharmacology and medication management to reduce readmissions will be reviewed and discussed.
