Course Description:

Lumbar traction is a physical therapy intervention for the conservative treatment of LBP. Despite publication of studies and clinical guidelines that indicate that there is little evidence to support the use of traction in the management of LBP, surveys show widespread continued use, and controversy persists. Current literature suggests that it might be effective for treatment of a subgroup of patients with LBP, and attempts have been made to identify the characteristics of patients who will benefit. It is commonly used for subacute LBP presenting with nerve root signs. Traction has been advocated to distract the facet joints, increase the space between the vertebrae, open intervertebral foramina, stretch joint capsule and ligaments, and reduce muscle spasm. Decreased size of herniated disc material with continuous lumbar traction has been documented. In general, traction is advocated as a component of a treatment program that includes other physical therapy methods rather than as a sole treatment modality. Variations in lumbar traction methods include mechanical or motorized traction, manual traction, auto-traction, inversion traction, extension traction, and vertical ambulatory traction.
