Course Description:

The role of therapists working with adults with chronic conditions has dramatically changed. To achieve positive long-term outcomes, therapists must adopt the role of "coach" by providing patients with tools to independently manage their condition or chronic disease. These are skills not formally taught but essential for success. This course describes the value of empowering your patients through a therapeutic alliance, how to integrate these concepts into your practice, and how to leverage community resources to create a continuum of care.
