The national and worldwide incidence and prevalence of diabetes has significantly increased over the last several decades, due in part to the “obesity epidemic.” Of special concern is the increased rate of diabetes among younger populations and the increased number of patients experiencing the consequences of “failed” wound healing, especially amputation and its associated increase in mortality.

Individuals with diabetes are at increased risk for a variety of serious and disabling complications. These complications often increase morbidity and mortality. The presence of diabetes increases both the risk for developing a chronic wound and in delayed healing once wounds are present. In recent years, researchers have made great progress in understanding the complex processes that facilitate and control normal wound healing (physiology) as well as the mediators that impair proper healing (pathophysiology) in diabetic wounds.

Members of the Wound Care Team should have a thorough understanding of both diabetes and its effects on wound healing. This will result in increased effectiveness in assessing both patients and wounds, in using a scientific or rational basis for determining comprehensive treatments, and in counseling both patients and caregivers. A thorough understanding will also aid Wound Care Team members in educating health professionals who may not routinely treat chronic wounds and those in nonclinical positions. The latter includes hospital or clinic administrators, insurance representatives, auditors, and various regulators.
