Course Description:

Practitioners are often educated on the general benefits of why an integrated approach to resident care results in best outcomes, but the application of interdisciplinary care is often unrealistic to include in daily practice due to various barriers, limited materials, and inconsistent messaging. The integrated team shares applicable assessments to collaborate on resident sensory perception, activity level, continence level, mobility, as well as other specialized treatments based on the needs presented. Integrated team collaboration is vital to provide faster access to the best care. Using evidence-based interventions and an integrated approach, all practitioners are able to efficiently and effectively identify residents at risk for falls, implement interventions to help reduce falls, ensure a focus on a safe environment and reduce the likelihood of injury from a fall, collaborate with key members of the interdisciplinary team in order to ensure consideration of the whole-person, manage falls effectively when they occur, evaluate the effectiveness of interventions through the care planning process and make changes as needed to prevent additional falls, and provide team member education, training, and resources in order to create a culture of safety and increased awareness of risk.
