Course Description:

Chronic, lifestyle related disease is an epidemic in the US. Adults over disproportionally
affected, with 85% living with multiple conditions. Health coaching has significant
evidence supporting its value in chronic disease management. This course will enable
physical therapists and occupational therapist to apply evidence based coaching
strategies, especially the emerging strategy of capacity coaching, to improve the quality
of life in older persons living with multiple conditions.
