Course Description:
As a member of the interdisciplinary team, physical and occupational therapy professionals play a critical role in helping to determine discharge recommendations from acute care. There are many factors therapists must account for, including the patient's clinical presentation, psycho-social factors, insurance, family goals, facility regulations, reimbursement, and even access to technology. Therapists must be able to integrate this information and recommend the appropriate setting where the patient will thrive. With this knowledge therapists can contribute to facilitating smooth transitions between levels of placement and care.
