Course Description:

This training will increase the clinician’s knowledge on the risk of potentially preventable hospitalizations including risk factors for rehospitalization. The learner will gain knowledge on assessment strategies, timely intervention, and goal setting to reduce our potentially preventable rehospitalization rate.
