Since the inception of value-based care in 2020 with the Patient-Driven Groupings Model (PDGM), a Home Health Physical Therapist's value has been challenged as a necessary role in the Home Health team. By utilizing evidence-based tests & measures, Home Health Physical Therapists can prove their value is as high as ever.
