Course Description:
This course addresses the models of survivorship care delivery employed by health care facilities that provide cancer care. The content includes a description of current models of delivery, the advantages and disadvantages of each, and strategies to address the barriers to delivery. The Commission on Cancer accreditation standards impacting cancer facilities and the care of cancer patients and survivors is presented. In particular, the importance of the treatment summary and survivorship care plan and the essential components of each are discussed. This content is appropriate for health care professionals whose practice involves the care of cancer survivors.
