Course Description:

Rehabilitation therapy providers have a proven role in preventing unplanned hospital readmissions. This module discusses the diagnoses, patient factors, and healthcare system factors that increase a person’s risk of readmission. The role of physical therapy, occupational therapy, and speech-language pathology professionals in overcoming these problems is explored using case examples.
