Course Description:
For patients, improper discharge planning can lead to suboptimal recovery, functional decline, and preventable complications. It also increases the risk of hospital readmissions, which can adversely impact patients’ health and quality of life. Ensuring patients receive the appropriate level of post-acute care is essential for supporting recovery, enhancing independence, and improving satisfaction with their care. This course provides a comprehensive exploration of post-acute rehabilitation options, with a focus on the various pathways available for patients requiring continued care after an acute hospital stay. Participants will gain a comprehensive understanding of the criteria for inpatient rehabilitation (IRF/IPR) eligibility and learn to identify patients who may benefit from this level of care. This course also covers the critical role of Physical Medicine & Rehabilitation (PM&R) in the recovery process, including the assessment, treatment and coordination of care for patients who have functional deficits following illness, injury or surgery.
