Course Description:
Currently available prognostic models are inadequate to support shared decision-making and care planning for clinicians, patients and family members, and for justification to administrators and payers. This course will highlight Mary Free Bed’s stroke trajectories implementation project and its efforts to implement routine patient-centered outcome evaluation to facilitate shared decision-making and care planning based on patient-level recovery predictions, and to evaluate value based on patient outcomes. The course emphasizes the collaborative roles of clinicians, patients, families, and other stakeholders in shaping and applying these models effectively. Participants will also critically evaluate the advantages and limitations of mobility and daily activity trajectory models using two widely adopted rehabilitation measurement instruments. Practical implications for clinical decision-making in stroke rehabilitation will also be discussed.
