Course Description:

This course provides an in-depth exploration of patient needs following a rehabilitation stay. Participants will analyze existing gaps in post-rehabilitation services and support systems to better understand challenges faced by patients transitioning from rehabilitation to community living. Additionally, the course equips learners with the knowledge and skills to identify, evaluate, and apply local and national community resources, empowering them to facilitate comprehensive care and support for patients beyond the rehabilitation setting.
