Course Description:

Managing fall risk in older adults with multiple comorbidities is a unique and significant challenge to the interdisciplinary team (IDT) in post-acute care settings. Through a series of practical case studies of complex patients, this course presents evidence-based best practices for an IDT to manage fall risk in patients with multiple medical fall risk factors. The process of root-cause analysis is reviewed, and interventions that include all members of the care team to improve outcomes are discussed.
